I recognize the importance of rendering personal service to my community in cooperation with other civic-minded persons. I understand that membership is not vaid until approved by the club's board of directors.

By checking this box, I acknowledge and agree to pay the applicable annual membership dues for the Silicon Valley Open Doors Lions Club: $200 per year for each adult Lion age 25 or older and $100 per year for each Lion under age 25. Dues are required upon enrollment for all new and transferring members and annually thereafter.
